How to file a Privacy Complaint

If you believe that DMH violated your (or someone else’s) health information privacy rights or committed another violation of the Privacy Rule, you may file a complaint with the Privacy Officer assigned to the specific DMH facility at which the violation may have occurred.

In addition, you may also file a complaint with the Secretary of the U.S. Department of Health and Human Services at:
